Centerforce supports, educates, and advocates for individuals, families and communities impacted by incarceration. We are currently seeking a dynamic individual to join our Positive Transitions (POST) Study. This position is specifically responsible for health education, motivational, client-centered risk reduction planning and development of prevention service plans pre- and post-release with HIV+ inmates from the California Medical Facility in Vacaville and the San Francisco County Jail.
Position Description:
- Health Education: Provide client-centered health education around behavioral risk-reduction to HIV+ inmates and recently released individuals using motivational interviewing techniques. Develop client-centered behavioral and clinical care goals following release.
- Prevention Education: Facilitate prevention service plans with HIV+ inmates and recently released individuals at the California Medical Facility and the San Francisco County Jail. Develop client prevention service plans based on clientÕs needs. Assist in successfully making linkages with prevention-focused community agencies with/on behalf of clients, providing ongoing post release educational support and keep detailed notes concerning all aspects of prevention case management services.
- Other duties: Position requires extensive travel around the San Francisco Bay Area and great attention to detail. Participation in weekly meetings, individual supervision and other project meetings as assigned. Other duties upon request and/or as needed.
Qualifications:
- B.A. in Social Work, Public Health, or related field with case management and/or individual health education or counseling experience;
- 3 years of experience in a health/social service organization and/or equivalent combination of education and experience;
- Must be familiar with HIV, Hepatitis, Sexually Transmitted Diseases and have experience delivering client-centered services;
- Strong organizational, communication skills;
- Ability to work on a variety of tasks simultaneously and on time;
- Ability to work as part of a multidisciplinary team with people from diverse ethnic, cultural, economic, sexual orientation, and gender identified communities required;
- Ability to work within the prison system
All program staff are required to undergo training and security clearance by the California Department of Corrections which requires the following information in order to complete the application process: full name, California drivers license #, social security #, and date of birth.
